7 Sep 2018 A medulloblastoma is a malignant primary brain tumor located in the posterior fossa near the fourth ventricle. It is the most common malignant
Medulloblastoma is the most common malignant pediatric brain tumor, accounting for 40% of childhood tumors in the posterior fossa. 1 Genomic characterization of medulloblastoma has recently demonstrated that medulloblastomas can be subdivided into 4 distinct molecular subgroups: wingless (WNT), sonic hedgehog (SHH), group 3, and group 4. 2 ⇓– 4 These subgroups have shown different clinical behaviors and may benefit from subgroup-specific treatments. DIAGNOSIS: 1&2: Cerebellar tumor: Medulloblastoma - Desmoplastic/nodular phenotype; Sonic Hedgehog molecular sub-type (see Comment). COMMENT: Immunohistochemistry performed on the original tumor showed absence of nuclear staining for Beta catenin and upregulation of YAP1 and GAB1 indicating a Sonic Hedgehog (SHH) molecular subtype. Objective: Medulloblastoma (MB) is a heterogenous tumor, and the prognosis is influenced by various clinical, histological, and molecular factors. The aim of the study is to determine the clinical profile and radiologic characteristics among the histo-molecular subgroups, the predictors of surgical outcome, and the pattern of relapse in pediatric and adult MB.
Group 3 tumors occur more commonly in males than females and are found in infants and children but almost never in adults. Tumor enhancement can be both homogeneous or heterogeneous. Similar to ependymoma, approximately14% of medulloblastomas may show foraminal extension.
They are found most commonly in adults and infants, but infrequently in children. Assoc Prof Frank Gaillard ◉ ◈ et al. Medulloblastoma - wingless (WNT) subgroup tumors are the least common medulloblastoma subgroup and have by far the best prognosis. They are usually seen in children and, to a lesser degree, adults.

Estimate the annual incidence of medulloblastoma (MB) in the U.S. What is its frequency relative to other CNS tumors in children? ~500 cases/yr of MB in the U.S. It is the 2nd most common pediatric CNS tumor (20% of cases; #1 is low-grade glioma at 35%–50%). It is the most common malignant brain tumor in the posterior fossa (PF) in MICROSCOPIC DESCRIPTION: 1&2: Paraffin sections show a densely hypercellular medulloblastoma involving cerebellum. Tumor cells have intermediate and large round and oval vesicular and hyperchromatic nuclei and a variable amount of pale cytoplasm.

They most commonly present as midline masses in the roof of the 4 th ventricle with associated mass effect and hydrocephalus. Very large mass located centrally within the posterior fossa. It is high T2 signal and low T1 signal with only patchy contrast enhancement. No normal vermis can be identified and the mass is likely to arise from it, completely effacing the fourth ventricle and abutting the dorsal aspect of the brainstem without convincing evidence of extension into the pons.
